Outreach Services

Outreach Services is available to visit you in your home to discuss services that
are provided by Senior Connections. 

For those individuals who would like to utilize one of our services, Outreach
Services can provide referrals to the Senior Connections Benefit Specialist,
Information and Assistance Specialist, and the Caregiver Program.  Outreach Services assist with home delivered meal assessments and provides information
on the Meals Program and Transportation.
